Acute coronary syndrom is the term used for the acute presentation of ischaemic cardiopathy. Its physiopathological mechanism is triggered first by the inestability of preexisting atheromatous plaques within the coronary arteries. And it's acute clinical expression varies between angina, heart attack and sudden death. In 1985 the terminology ACS (acute cronary syndrom) was introduced and its clasification in NSTE-ACS or STEMI (depending on the ECG features) gives us the possibility of a direct correlation between the underlying pathophysiological mechansim and the acute event. Detecting and understanding this mechanism is essential to quickly and confidently decide the best managment of our patients. We are provided by other tools which can help us in the diagnosis and treatment of these patients, as examples: serum markers of myocardial injury, coronariography and other imaging techniques.
